Enrollment Process

To enroll your student/self, click on the K-12 Enrollment or the Adult Enrollment link and complete the application. Then, email, fax, text, or mail all of the necessary forms listed below.

•  Enrollment packet (K-12 and Adult Learners)
•  Birth Certificate (K-12)
•  Immunization Records (K-12 and Adult Learners)
•  Proof of residence (K-12 and Adult Learners) 
   Utility bill or equivalent no more than two months old.
   Financial institution documents such as a bank statement, deed, or mortgage with a current Kansas address.
   Rent or lease agreement dated within the past 12 months.
   Kansas Voter Registration Card.
   Current vehicle registration.
   Current Kansas Driver’s License or I.D. renewal postcard
•  Proof of ID (Parent/Guardian of K-12 or Adult Learner)
•  Health Assessment form (Kindergarten)
•  Special service plan if applicable to child: 504, IEP (K-12)

 Once you have filled out and submitted your enrollment form, KOLP will contact you by email.
